Making the hospital transition easier.
Leaving the hospital, espcially after emergency teatment, can be overwhelming. It can be even more difficult if the elderly person cannot return home, either temporarily or permanently, and needs an assisted living facility or a nursing home. A new home may have to be found - and quickly. A senior day care center or senior home care may be other options. An experienced professional, such as Geriatric Care Manager with knowedge of local facilities and services, may be able to make the situation less stressful and confusing for all concerned. Other support services, or even advice from support groups may also be helpful. Financial, legal and insurance status may also need to be reviewed at this time, and public assistance options shold be investigated. This may also be the time to look at future issurs such as nutrition, source for needed medical supplies, transportation services, etc.
